  1. Education: Graduate from a school of pharmacy or possess an active license as proof of education – Required.
  2. Preferably: Doctor of Pharmacy – Preferred.
  3. Experience: Minimum 1 year of hospital pharmacy experience or completion of PGY1 residency – Preferred.
  4. Skills: Critical thinking abilities, decisive judgement, and the capability to work with minimal supervision. Must excel in a dynamic environment and take appropriate action.
  5. Certifications: Basic Life Support (BLS) obtained within 10 days of hire; Advanced Cardiovascular Life Support (ACLS) obtained within 30 days of hire; LICENSE: State Board of Pharmacy in good standing.

Essential Functions:

  • Oversee drug therapy protocols as part of an interdisciplinary team.
  • Evaluate medication orders for accuracy and completeness prior to order entry.
  • Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
  • Document interventions and outcomes for use in drug therapy quality improvement efforts.
  • Manage patients in an outpatient anticoagulation clinic, including operation of Point-of-Care testing, evaluation of results, dosing, and scheduling.
  • Maintain adequate knowledge of pharmacy automated systems and computer software.

Shifts: Either (5) 8-hour shifts or (4) 10-hour shifts, weekends will include one major and 1-2 minor holidays, and backfill if needed for vacation fill-in. 1-2 days of call per week. Very rarely needed to come on site, just an occasional phone call.
