Our client, a leading healthcare provider, is actively seeking a dedicated Occupational Therapist to join their dynamic team. This award-winning organization is recognized for its commitment to providing exceptional patient-centered care across various locations.
Conduct thorough assessments in Occupational Therapy to identify participant needs, preferences, and goals.
Collaborate with the interdisciplinary team (IDT) to develop comprehensive care plans, ensuring participant focus and values guide the process.
Deliver and document Occupational Therapy interventions aligned with physician directives and participant care plans.
Provide supervision to Therapy Assistants, offering training and guidance for the implementation of Functional Maintenance Programs (FMP).
Conduct training sessions for staff, participants, and caregivers in relevant areas of Occupational Therapy.
Collaborate with IDT members to facilitate smooth care transitions between settings and actively participate in end-of-life care coordination and support.
Bachelor’s Degree in Occupational Therapy from an Accredited Program following AOTA & ACOTE standards.
Two (2) years of clinical experience in occupational therapy, specializing in a frail or elderly population.
Demonstrated supervisory experience with the ability to mentor and develop team members.
Registration from the National Board for Certification in Occupational Therapy.
Licensed and/or eligible for licensure as required in the state of employment.
