As the Medical Director, reporting to the Chief Clinical Officer, you will play a pivotal role in the IDT, delivering care within our client’s PACE program. This opportunity is ideal for a clinician who values meaningful patient interactions without the misaligned incentives of fee-for-service.

Essential Job Duties:

  • Collaborate with the IDT to conduct comprehensive assessments and develop individualized care plans for participants.
  • Actively engage in the IDT, communicating participant changes, contributing to care planning arrangements, and coordinating 24-hour care delivery.
  • Work closely with network providers and the IDT to ensure effective care coordination, especially around hospital discharge, short-term, and long-term placement.
  • Provide guidance to Nurse Practitioners.
  • Oversee participants’ utilization of medical specialists and inpatient care.
  • Participate in rotating night and weekend calls.
  • Supervise all participant care in nursing homes, ensuring regular visits per PACE standards and participants’ needs.
  • Review outcome and utilization data with the IDT to identify opportunities for improved care and operations.

Job Requirements:

  • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
  • Master’s/Doctorate degree.
  • Board Certification in Internal Medicine or Family Practice.
  • Preferred Board Certification in Geriatrics.
  • Active California Physician License without restrictions.
  • State CDS Certification and DEA Registration.
  • Valid state driver’s license with a clean DMV record and recent vehicle insurance.
  • Excellent clinical, organizational, and communication skills in frail, elderly participant settings.
  • Current knowledge of chronic care/geriatric issues and best practices.
