Award-Winning Client is currently seeking a highly motivated Integrated Pharmacist to join their dynamic team. Our client has a strong presence across various locations and is recognized for excellence in healthcare.
Position Overview: Our client is looking for a dedicated individual with a Clinical Pharmacy Residency, PGY1 minimum, and Clinical Pharmacy Specialty experience in General Pediatrics, NICU, or PICU. The successful candidate will be responsible for various tasks, including filling and labeling prescriptions, dispensing controlled substances, and participating in the Medication Use Evaluation Program.
Minimum Qualifications:
- Graduation from an accredited School of Pharmacy.
- Licensed as a Registered Pharmacist in the State of [State]; Pharmacy Sterile Product certification through an in-house program or other accredited program is required if the incumbent compounds sterile products.
- Two (2) years of experience as a hospital Pharmacist (or at the discretion of System Executive, System Pharmacy) with knowledge of the unit dose medication and I.V. admixture processes.
- Demonstrates commitment to the Partners-in-Caring process by integrating our culture in all internal and external customer interactions.
Responsibilities:
- Fill and label prescriptions, providing drug-related information and education for in-patients, out-patients, hospital employees, and medical staff.
- Dispense controlled substances and maintain appropriate inventory records.
- Supervise the activities of the pharmacy technical staff and mentor or train pharmacists, pharmacy students, or residents.
- Participate in the Medication Use Evaluation Program and the Process Improvement Program.
- Maintain medication profiles, review profiles for drug-related problems, and provide consultation on drug research.
- Maintain the scheduling and preparation of I.V. solutions and additives.
- Initiate reports of medication errors or adverse drug interactions.
- Assume full responsibility for department management in the absence of the Pharmacy Manager or Lead Pharmacist.
- Work with other professionals to generate newsletters, draft policies or procedures for review by the Pharmacy Director.
- Perform clinical interventions, including medication profile review, IV to oral conversion, renal dosing adjustment, antimicrobial streamlining, patient counseling, obtaining medication histories, vancomycin/aminoglycoside dosing, therapeutic drug monitoring, and participation in multidisciplinary rounds.
- Document clinical interventions.
