- Collaborate with interdisciplinary teams to ensure effective patient care coordination.
- Participate in care management processes, emphasizing patient self-determination, social determinants of health, and the complexities of healthcare.
- Utilize your healthcare expertise to facilitate communication and care coordination along the patient’s journey.
- Implement transitional care management strategies to enhance patient outcomes.
- Leverage your nursing skills to provide care in various healthcare settings, such as acute, ambulatory, or post-acute environments.
- Engage in Case Management, including Care Coordination and Utilization Management, while adhering to the Transitions in Practice (TIP) program guidelines. TIP candidates should have experience in the nursing unit relevant to the Care Manager position.
- Bachelor’s Degree or equivalent education/experience.
- Possession of a California Registered Nurse License upon hire.
- Bachelor’s degree in nursing.
- Master’s degree in a healthcare-related field.
- Certification in Case Management (ACM or CCM) upon hire.
