  • Master’s Degree in Social Work from an accredited program.
  • A minimum of one year of experience in healthcare-related roles, which includes completing an internship in a healthcare setting.
  • One year of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
